This new healthcare delivery model is built on lessons learned from the original Comprehensive Primary Care Model. Supporting the move toward value-based care, this initiative is another stepping stone to innovating care delivery and reforming payment structure in the United States.
The new CPC+ Model includes two different tracks that practices may apply, Track 1 and Track 2. Track 1 targets practices that have basic health information technology and the infrastructure necessary to deliver comprehensive primary care; the focus is on implementing and developing comprehensive primary care capabilities*. Track 2 targets practices that are proficient in primary care that are ready to increase the depth, breadth, and scope of medical care delivered to their patients, especially those with complex needs*.
Given eQHealth’s core competencies, our focus is on Track 2 that specifically requires more enhanced IT capabilities to aid in achieving CMS defined objectives, milestones and quality care goals. Those practices interested in applying to participate in Track 2 during the application period, August 1, 2016 - September 15, 2016, must have identified a Health IT vendor who will support the required IT capabilities when their application is submitted to qualify for participation. eQHealth is here to help.
